Re-issuance of Participation Certificate

The employer can apply to the Trustee for re-issuance of Participation Certificate if one of the following events happens:

(i) loss of Participation Certificate
(ii) damage of Participation Certificate
(iii) printing error or incorrect information

The employer should provide the following document(s) for re-issuance of Participation Certificate:

Reasons for Re-issuance of Participation Certificate Document Required
Loss of Participation Certificate
  • Photocopy of police report of loss
Damage of Participation Certificate
  • Original copy of damaged Participation Certificate, if not possible, please state the reason of damage.
Printing error / Incorrect information reported
  • Original copy of Participation Certificate
  • Details of the amendments required
